Per 17mei 2023maken wij gebruik van een nieuw patiëntportaal; “MijnGezondheid.net”. De App MedGemak hoort hierbij. Dit omdat wij overgegaan zijn naar een andere huisartsensoftware en dit patientportaal hoort daarbij.
Via de app van het nieuwe patiëntportaal herhaalt u chronische medicatie, stuurt u berichten of een e-consul naar de arts of assistente of POH en maakt u een afspraak met de arts.
Als u via een webbrowser inlogt u in het patientportaal heeft u inzage in uw dossier, inclusief bloeduitslagen, brieven van specialisten en kweek uitslagen. Oók kan u op deze manier een verwijzing of machtiging uit uw dossier downloaden. Dit hoeft dan niet meer apart gemaild of opgehaald te worden.

Met het in gebruik nemen van het nieuwe patiëntportaal, vervalt ons oude patiëntportaal bij “UwZorgOnline”.
Voor patiënten die zijningeschrevenbij Huisartsenpraktijk Statenkwartier zijn wij via onspatiëntportaalonline 24 uur per dag beschikbaar. Via het portaal kunt u zelf afspraken maken, herhaalrecepten aanvragen en korte vragen stellen aan de huisarts (e-consult). U bent hierdoor niet gebonden aan onze openingstijden en voorkomt wachttijden aan de telefoon. Ook heeft u inzicht in uw patiëntdossier met registratie van contactmomenten, bloed,- urine en foto-uitslagen en ontvangen berichten van uw specialist.
Eenmalig aanmelden
Om gebruik te maken van het patiëntportaal moet u zich eenmalig aanmelden. Dit gaat via DigiD.
Afspraken maken
U ziet welke plekken er nog beschikbaar zijn in de agenda van de verschillende artsen. U kunt hier zelf een afspraak inplannen op het moment van uw voorkeur. U ontvangt een automatische bevestiging van de gemaakte afspraak.
Het maken van online afspraken is uitsluitend geschikt voor enkelvoudige (korte) klachten die niet spoedeisend zijn. Mocht u langer dan 10 minuten nodig hebben of meerdere klachten willen bespreken, neemt u dan contact op met de assistente. Ook voor vragen die spoedeisend zijn (of u twijfelt hierover) adviseren wij u om contact op te nemen met de assistente.
Let op: tot 1 dag van tevoren kan u een afspraak inplannen en annuleren, op de dag zelf moet u dit via de assistente regelen.
Ook voor bloedprikken kan u nu zelf een afspraak maken. Let wel op: dan moet er een aanvraagformulier gemaakt zijn door de arts of praktijkondersteuner!
Herhaalmedicatie aanvragen
In het online portaal kunt u medicatie die eerder door ons is voorgeschreven en u vaker gebruikt zelfstandig herhalen.
E-consulten
Via deze optie kunt uvragen stellen aan een van onze huisartsen. U kunt dan denken aan vragen over uitslagen, medicatie gebruik, follow up na een eerder consult e.d. Het e-consult is zeker niet geschikt voor zaken die spoedeisend zijn of waarbij de arts u lichamelijk moet onderzoeken.
Indien u twijfelt over de ernst van de klacht, neem dan altijd telefonisch contact met de praktijk op.

As of May 17, 2023, we will be using a new patient portal called “MijnGezondheid.net” along with the MedGemak app. This is because we have transitioned to a different general practitioner software, and this patient portal is part of that transition.
Through the app of the new patient portal, you can request repeat prescriptions for chronic medication, send messages or e-consultations to the doctor, assistant, or healthcare assistant (POH), and schedule appointments with the doctor.
When you log in to the patient portal through a web browser, you can access your medical record, including blood test results, letters from specialists, and culture test results. You can also download referrals or authorizations from your record in this way. This eliminates the need for separate emails or pickups.

With the implementation of the new patient portal, our old patient portal at “UwZorgOnline” will no longer be available.
For patients registered with Huisartsenpraktijk Statenkwartier, we are available online 24 hours a day through our patient portal. Through the portal, you can schedule appointments, request repeat prescriptions, and ask brief questions to the doctor (e-consult). This way, you are not bound by our opening hours and can avoid waiting times on the phone. You also have access to your patient record, which includes a registration of contact moments, blood, urine, and photo results, as well as received messages from your specialist.
One-time registration
To use the patient portal, you need to register once. This is done through DigiD.
Scheduling Appointments
You can see the available time slots in the agendas of different doctors. You can schedule an appointment at your preferred time. You will receive an automatic confirmation of the appointment made.
Scheduling appointments online is only suitable for simple (short) complaints that are not urgent. If you need more than 10 minutes or have multiple complaints to discuss, please contact the assistant. We also advise you to contact the assistant for urgent questions (or if you are unsure).
Please note: You can schedule and cancel appointments up to 1 day in advance. On the same day, you need to arrange this through the assistant.
You can now also schedule appointments for blood tests. Please note that an requisition form must be created by the doctor or healthcare assistant.
Requesting Repeat Medication
In the online portal, you can independently request medication that has been prescribed by us before and that you use regularly.
E-consultations
Through this option, you can ask questions to one of our general practitioners. This can include questions about test results, medication use, follow-up after a previous consultation, etc. The e-consultation is definitely not suitable for urgent matters or situations where the doctor needs to examine you physically.
If you are unsure about the severity of your complaint, always contact the practice by phone.
