Per 5 mei 2021 maken wij gebruik van een nieuw patiëntportaal; “Uw Zorg Online”.
Via het nieuwe patiëntportaal heeft u inzage in uw dossier, herhaalt u chronische medicatie, stuurt u berichten of een e-consult en maakt u een afspraak met de arts.
Het patiëntportaal kan u via een app of via een website bereiken: https://huisartsenpraktijkstatenkwartier.praktijkinfo.nl/patientportaal/
Voor de app kiest u hiervoor deUw Zorg Online appin deApp Store(iOS) ofGoogle Play(Android).
Met het in gebruik nemen van het nieuwe patiëntportaal, vervalt ons oude patiëntportaal bij “Praktijkinfo”.
Voor patiënten die zijningeschrevenbij Huisartsenpraktijk Statenkwartier zijn wij via onspatiëntportaalonline 24 uur per dag beschikbaar. Via het portaal kunt u zelf afspraken maken, herhaalrecepten aanvragen en korte vragen stellen aan de huisarts (e-consult). U bent hierdoor niet gebonden aan onze openingstijden en voorkomt wachttijden aan de telefoon. Ook heeft u inzicht in uw patiëntdossier met registratie van contactmomenten, bloed,- urine en foto-uitslagen en ontvangen berichten van uw specialist.
Eenmalig aanmelden
Om gebruik te maken van het patiëntportaal moet u zich eenmalig aanmelden. Aanmelden is alleen mogelijk voor patiënten die staan ingeschreven in de praktijk. Daarnaast moet er gebruik worden gemaakt van een uniek emailadres. Het is niet mogelijk meerdere accounts aan eenzelfde emailadres te koppelen.
Na aanmelding worden uw gegevens gekoppeld aan ons huisartsen-informatiesysteem. In verband met het belang van de privacy controleren wij alle koppelingen. Zodra deze koppeling klaar, en door ons bevestigd is, ontvangt u een bevestigingsmail en kunt u aan de slag.
N.B. Bij het aanvragen van het account wordt u gevraagd om uw adresgegevens en emailadres. Het is prettig als deze overeenkomen met de gegevens die bij ons bekend zijn. Geef eventuele wijzigingen zoals verhuizingen daarom altijd door aan de praktijk.
Afspraken maken
Via de webagenda ziet u welke plekken er nog beschikbaar zijn in de agenda van de verschillende artsen. U kunt hier zelf een afspraak inplannen op het moment van uw voorkeur. U ontvangt een automatische bevestiging van de gemaakte afspraak.
Het maken van online afspraken is uitsluitend geschikt voor enkelvoudige (korte) klachten die niet spoedeisend zijn. Mocht u langer dan 10 minuten nodig hebben of meerdere klachten willen bespreken, neemt u dan contact op met de assistente. Ook voor vragen die spoedeisend zijn (of u twijfelt hierover) adviseren wij u om contact op te nemen met de assistente.
Let op: tot 1 dag van tevoren kan u een afspraak inplannen en annuleren, op de dag zelf moet u dit via de assistente regelen.
Herhaalmedicatie aanvragen
In het online portaal kunt u medicatie die eerder door ons is voorgeschreven en u vaker gebruikt zelfstandig herhalen. Als u ingelogd bent in uw account ziet u een overzicht van de bij ons bekende medicatie die herhaald kan worden. U selecteert hier zelf de medicatie die u wilt herhalen. Door te kiezen voor “recept aanvragen” bevestigt u de keuze voor het te herhalen medicijn.
U ontvangt een bevestiging van de door u herhaalde medicatie.
E-consulten
Via de optie “vragen stellen” kunt u korte, NIET dringende,vragen stellen aan een van onze huisartsen. U kunt dan denken aan vragen over uitslagen, medicatie gebruik, follow up na een eerder consult e.d. Het e-consult is zeker niet geschikt voor zaken die spoedeisend zijn of waarbij de arts u lichamelijk moet onderzoeken.
Indien u twijfelt over de ernst van de klacht, neem dan altijd telefonisch contact met de praktijk op.

Via the new patient portal you can view your file, repeat chronic medication, send messages or an e-consultation and make an appointment with the doctor.

With the commissioning of the new patient portal, our old patient portal will expire at “Praktijkinfo”.
We are available online 24 hours a day for patients who are registered with the Statenkwartier GP practice through our patient portal. Via the portal you can make appointments yourself, request repeat prescriptions and ask short questions to the doctor (e-consultation). You are therefore not bound by our opening hours and avoid waiting times on the telephone. You also have insight into your patient file with registration of contact moments, blood, urine and photo results and receive messages from your specialist.
Sign up once
You must log in once to use the patient portal. Registration is only possible for patients who are registered in the practice. In addition, a unique email address must be used. It is not possible to link multiple accounts to the same email address.
After registration, your data will be linked to our GP information system. In the interest of privacy, we check all links. As soon as this link is ready and confirmed by us, you will receive a confirmation email and you can get started.
N.B. When applying for the account you will be asked for your address details and email address. It is nice if these correspond with the information that is known to us. Therefore, always pass on any changes such as relocations to the practice.
Make appointments
Via the web agenda you can see which places are still available in the agenda of the different doctors. You can schedule an appointment here at the time of your preference. You will receive an automatic confirmation of the appointment made.
Making online appointments is only suitable for single (short) complaints that are not urgent. If you need more than 10 minutes or want to discuss multiple complaints, please contact the assistant. We also advise you to contact the assistant for urgent questions (or if you are unsure about this).
Please note: you can schedule and cancel an appointment up to 1 day in advance, on the day itself you must arrange this through the assistant.
Request repeat medication
In the online portal you can independently repeat medication that has previously been prescribed by us and you use more often. If you are logged into your account, you will see an overview of the medications known to us that can be repeated. You select the medication you want to repeat here. By choosing “request prescription” you confirm the choice of the drug to be repeated.
You will receive confirmation of your repeated medication.
E-consultations
Via the option “ask questions” you can ask short, NON urgent, questions to one of our GPs. You can think of questions about results, medication use, follow-up after a previous consultation, etc.
The e-consultation is certainly not suitable for matters that are urgent or where the doctor must examine you physically.
If you are unsure about the seriousness of the complaint, always contact the practice by telephone.
Also via the app
You can reach our patient portal via the link on our page. You can also use the portal via your smartphone. You choose the Uw Zorg Online app in the App Store (iOS) or Google Play (Android).
With the commissioning of the new patient portal, our old patient portal via “Praktijkinfo” will expire. You must register separately for the patient portal.
